I am employed by Essex Police Authority as a Station Office Assistant, at Westcliff Police Station. On Monday 131195 I was on duty at Westcliff Police Station.

My tour of duty was 1100 hours to 1900 hours. At 1630 hours on this day I was in the front office when a male person came in to produce his driving licence car insurance and MOT test certificate.

He stated his full name was Jimmy Lawrence KEBBELL and he lived at 17 Guildford Road, Southend, Essex. I recorded his full details on form HO/RT2.

I produce this reference JAD/1. He made his signature on the same form having produced his driving licence. I took possession of his driving licence and gave him a receipt for the same, which I signed.

This receipt is the bottom part of the actual fixed penalty form which I recorded as No 6806370-4 on the HO/RT2 form. The male then left the front office.

I remember I was busy that day and shortly after the male left the front office, I went put the driving licence into the envelope to send to the Central Ticket Office, and realised it was missing.

I placed a note to explain mislaying the driving licence on the copy of the HO/RT2. I produce a copy of the note reference JAD/2.

I searched the front office area immediately and could not find the driving licence anywhere. I have since looked at the HO/RT2 book and can see that the next entry was at 1700 hours, and a person by the name of Patrick Terrence TATE produced his documents.

I produce a copy of this entry reference exhibit JAD/3. I have since obtained a photocopy of the original fixed penalty ticket for KEBBELL which I produce JAD/4.
