PART 1
If the driver is not the person to whom the letter was
sent and is known to you then they may complete the
form in your presence and you must return it.

2 OR 3 OF THE FORM

licence.
Your driver number is shown on the front of your licence
and contains part of your surname i.e. SMITH-416117-JD9??
If you are the driver and have lost or mislaid your
licence then please indicate this above the boxes. Ensure
that you apply for a new licence from DVLA straight
away as it will have to be produced to the court.
If you have not got a British Driving licence issued
by DVLA please state in the boxes NO BRITISH LICENCE.
Ensure you sign, and date, the form OR it will be not
be accepted.
PART 2
If the driver has filled in part 1 DO NOT FILL IN THIS
PART AT ALL.
This part is for the nominated driver who was driving
your vehicle at the time of the offence. If you have
a phone number that can be used please add this to the
address line.
Please complete with the full name, address and post
code of the driver.
PART 3
If you have sold or disposed of the vehicle then this
part of the form must be filled in. Please ensure you
enter you disposed of the vehicle and have informed
DVLA. Failure to notify DVLA of disposal is an offence.
HIRE OR LEASED If the vehicle is either hired or leased
by you to another person or company you must complete
this part. Please enclose a copy of the hire agreement.
Examination of Photographic Evidence.
This may be done by prior appointment with the Camera
Enforcement Office by telephoning 01277- 636610 between
9.30 am and 3pm Monday - Friday quoting your reference
number Q104/02/0049.
